Fractional operator · Regulated healthcare

Two engagements at a time. Here's how they work.

I'm a fractional COO for US telehealth, diagnostics, and pharmacy companies scaling from $5M to $150M. I come in when operations, AI and regulation start pulling on the same thread. I keep it to two clients at a time, because this work needs a head that isn't split five ways.

01

The Diagnostic

$7,500

Fixed fee · 2 weeks

A teardown of how your operation actually runs (intake to fulfillment, the AI in the loop, where the regulatory exposure sits). You get a gap hypothesis chained to evidence and a 30-60-90 plan your team can execute with me or without me.

Continue into a retainer within 30 days and the full $7,500 goes toward month one.

02

Fractional COO

$12,000/ month, from

1 to 2 days a week · 3-month minimum

Most engagements land between $12K and $18K, depending on scope. I own the plan from the diagnostic, run the operating cadence, and build the systems (SOPs, KPI dashboards, AI with guardrails) that keep running when I step back.

03

When fractional should become full-time

Sometimes the honest answer after 90 days is that you need a full-time COO. For the right team, that can be me. By then we'll both know.

Questions buyers ask

What does a fractional operator do in healthcare?

A fractional operator runs part of the business as a senior executive, on a retainer, for a few days a month. In healthcare that means owning the operating layer between patients, clinicians, partners and regulators (intake, fulfillment, the AI in the loop and the audit trail behind it).

How much does a fractional COO cost?

In my practice, a 2-week diagnostic is $7,500 and the retainer starts at $12,000 a month (most land between $12K and $18K). You get a senior operator now, without the salary, equity and months-long search a full-time hire takes.

What happens after the diagnostic?

You keep the 30-60-90 plan either way. If you want me to run it, we start a retainer within 30 days and the $7,500 is credited to month one. If your team runs it, the plan is written so they can.
